SUBMIT EMPLOYEE CENSUS

You should include the following information for each employee:

  • First & Last Name
  • Date of Birth
  • Gender
  • Zip Code

Please also include the same information for any dependents who will be covered.
(employee names are not required at this time for census)

PROVIDE BUSINESS INFORMATION

This includes the following:

  • Legal Name & DBA
  • Address
  • Contact Person
  • Type of Business (Corporation, LLC, Sole Proprietorship, etc.) SIC Code
  • # of Years in Business
  • Coverage Desired (Medical, Dental, Vision, Life, Etc.) Current Insurance Coverage
  • Benefits
  • Premiums

*Information on current coverage is not necessary but but would help us in providing a plan that would make sure you and your employees benefits would stay the same.

OBTAIN QUOTE FOR BASE RATES

These rates will provide a starting point, but they will change based on information provided in next step.

PRESENTATION OF OUR PLAN AND OPTIONS

Once your base rate quote is ready, your representative will be in direct contact with you to discuss your initial plan benefits to make sure we are providing you a plan that is not only equal to what you have, but exceed what you presently have for less premium dollars.
